Families often turn to placement professionals and care managers when they are trying to understand what kind of support will work in real life. The Premier Partner Network gives qualified professionals a structured way to bring Total Care Connections into those conversations when home care may be appropriate. You help identify the opportunity and connect the family. Total Care handles the assessment, care plan, staffing, billing, and delivery of care. The goal is a long-term working relationship built around helping families get the support they actually need.

Identify a qualified opportunity. A family may need private-pay home care, ongoing daily support, help after rehabilitation, or additional care to remain safely at home or in their current senior living environment.

Connect with Total Care. Premier Partners use the designated partner communication process to share new opportunities and coordinate next steps.

Help schedule the care assessment. The goal is more than submitting a name. The partner helps move the conversation toward a care assessment with the family.

Total Care meets with the family. A Total Care representative completes the assessment, discusses the family’s needs, recommends a care plan, and guides the start-of-care process.

Stay involved when it helps. The referring partner may attend the assessment when the family consents and the partner’s presence supports the conversation.

Build your Home Care Revenue Portfolio. When an eligible private-pay client begins service, that client is added to the partner’s Home Care Revenue Portfolio and monthly commission reporting begins.

Premier Partners are part of an active network with ongoing education, practical updates, shared wins, and monthly recognition. Program materials include monthly leaderboards for Most Starts of Care and Highest Monthly Billable Hours.

What generally qualifies for commission
Commission eligibility is based on the type of care, service schedule, payer source, and completion of program requirements.
Eligible commissionable referrals must generally be private pay, require 20 or more hours of care per week, be scheduled in shifts of at least four hours, complete a Total Care assessment, and complete applicable partner disclosure and consent requirements. Whenever possible, the family decision-maker should participate in the assessment.
Government-payer referrals, including Medicaid, ALTCS, VA, Medicare, TRICARE, CHAMPVA, and similar benefit-based referrals, are welcome but are non-commissionable under program requirements. A completed partner agreement, W-9, and BAA are required before commission payments are issued.
Program terms, commission eligibility, and payment requirements are subject to the Premier Partner agreement and applicable requirements.
The Premier Partner Network is limited to 10 Premier Partner seats in each market. Once those seats are filled, that market closes to additional Premier Partners. The purpose is to build a manageable group of active partners who know the program, understand the care process, and want to work with Total Care Connections over the long term.
